Care is needed Monday - Sunday between the hours of 10: 00 am - 4:00 pm and caregiver should be able to start on ASAP. This Job # 494288 & 494289 is approximately 4 hours per week. You will play a pivotal role in relaying information back to the health plan using digital care plans. The primary duties of this caregiver will include Personal Care - Bathing, Grooming, Toileting, Transferring & Mobility - use walker, wheelchair and cane - last fall in March - fall risk, Medication Reminders, Companionship & Communication.
