CareLinx and United Healthcare (UHC) are partnering to provide thousands of members with in-home care benefits and are looking for a caregiver near Lombard, IL. Care is needed 1 visit per week for 3 hrs shift from 2pm, we will set a schedule with the caregiver and the caregiver should be able to start on ASAP. This job is approximately 3 hours per week - Mr has 8 hrs each month and Mrs has 8 hrs per month. You will play a pivotal role in relaying information back to UHC through the use of digital care plans. The primary duties of this caregiver will include: Her needs: Companionship, Transportation - dr appts, rx, groc/shop, Meal prep - no pork or beef, low sodium, His needs: Companionship, Bathing - H, Grooming - help with brushing his teeth, dress/undress, Toileting - H, Transfer/Mobility, Exercise, Transportation - dr appts, rx, groc/shop, Meal prep - low sodium, no pork no beef, Med Reminders.
