The following are common duties which include but are not limited to: * Monitor patient's vitals and record them in their file * Maintain the cleanliness and comfort of each patient, including bathing, feeding, and dressing * Assist nursing and medical staff with basic medical tasks, such as administering medication * Update patient records in a confidential manner * Evaluate the patient's wellbeing, reporting any needs to medical staff * Explain treatments and procedures to patients and families * Change dressings and bandages, ensuring patients are healing properly * Move patients from wheelchairs, beds and gurneys * Basic wound care including cleaning and bandaging injured areas * Giving injections of medications * Providing immunizations * Entering information into computer systems * Providing oversight to Nursing Assistive Personnel (NAP) * Monitoring fluid/food intake and output * Moving patients safely * Observing patient reactions to medications * Observing patient mental health status * Providing emotional support * Assisting with daily needs such as bathing and dressing Observing of patients for skin breakdown

Working closely with the Lead, my responsibilities are amongst others, listed below: * Assisting with personal care: bathing and grooming, dressing, toileting, and exercise * Basic food preparation: preparing meals, shopping, housekeeping, laundry, and other errands * General health care: overseeing medication and prescriptions usage, appointment reminders and administering medicine * Mobility assistance: help with getting in and out of a wheelchair, car or shower * Personal supervision: providing constant companionship and general supervision * Transportation: driving to and from activities, running errands, and help getting in and out of wheelchair-accessible vehicle * Emotional support: being a stable companion and supporter in all matters personal, health-related and emotional * Care for the elderly: orienting or grounding someone with Alzheimer's disease or dementia, relaying information from a doctor to family members * Back-up care (or respite) services: providing other caregivers a break * Home organization: help with organizing, packing or cleaning for a trip, or general house care and cleaning * Health monitoring: following a care plan and noticing any changes in the individual's health, recording and reporting any differences
