-Maintaining the confidentiality of the patient, and facility per HIPPA gidelines -Contributing to patient care based on provider directed orders and nursing assessment. -Admitting residents to faciltiy: Initiating assessments/care plans, clarifying and carrying out orders per MD to continue with. -Providing total patient care for; Long term, Geriatric, Hospice, Stroke, and Trauma Patients -Administering medications and vaccines via; PO, IM, SubQ, G-tube, Topical & Ear/Eyes -Assisting physicians during rounds -Setting up appointments and consultations based off residents plan of care -Supervising and delegating duties to non-licensed nursing staff -Performing ongoing monitoring of residents -Reporting findings to RNs and Physicians -Accurately inputting orders and documentation for patient care -Corresponding with healthcare professionals regarding patient care -Communicating effectively with patients, families, visitors and other staff members -Performing procedures:foleycath insertion, wound care, and suctioning -Implementing infection control during patient care. -Adhering to safety rules and regulations effectively, with equipment, and while carrying out duties, -Following through with discharging residents

-Maintaining the confidentiality of the patient per HIPPA guidelines -Implementing current nursing practice following a patient assessment and an approved plan of care initiated by the RN. -Providing patient care for; Geriatric/Hospice Patients -Adhering to safety rules and regulations effectively, with equipment, and while carrying out duties. -Implementing infection control during patient care. -Ensuring all patient DME's are working properly -Administering medications per Physician orders -Accurately inputting orders and documentation for patient care -Corresponding with healthcare professionals regarding patient care -Assessing and providing patient and family/caregiver education and information pertinent to diagnosis and self plan of care. -Providing appropriate pain/symptom management and evaluating a patient's response to treatments/medications. -Performing procedures:foleycath insertion, wound care, and suctioning if needed -Reporting findings to RNs and Physicians -Providing accurate and timely documentation of patient services
