Direct Service Provider – Support the development of independent living skills and promote choices while assisting person with intellectual and / or developmental disabilities (I/DD) to live productive and meaningful lives. Support includes personal care, assisting with home maintenance, mentorship, providing community connections and supporting employment. Respond to emergencies appropriately based on First Aid/CPR Training. Duties include administration of medication, completing monthly vitals, scheduling and being liaison for doctor visits, attending to personal needs, implementing Individual Service and Support Plans, completing necessary documentation, and planning and facilitating community activities with individuals. Assist with individual’s behavioral management needs and crisis intervention. Monitoring and ensuring safety, meal preparation, and home maintenance.
