Provides rehabilitation counseling and guidance to determine eligibility for services and to plan, develop, implement and manage a vocationally focused plan for employment for eligible individuals resulting in successful employment outcomes Manages cases through the rehabilitation process (referral to case closure) Enters data into the case management system and documents to reflect case activity and services provided Plans, authorizes and provides services in compliance with established policies and procedures Provides case management services to ensure correct and timely service provision Works directly with potentially eligible students, applicants and clients following policies and procedures

Monitors court cases in order to render assistance as appropriate *Identifies and assesses the needs of victims *Coordinates the assistance or service to victims of violence *Works alongside prosecutors and other Solicitor's Office staff *Establishes liaison with other police agencies, governmental agencies and social service agencies *Personally assists victims by providing direct victim services *Advises victims of their rights and remedies *Prepares and timely submits required reports *Maintains a complete record system of victims *Performs training to various groups on direct victim services *Assists crime victims with filling out Crime Compensation Forms *Coordinates and networks with other crime victim advocacy programs *Performs related duties as assigned *Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions

Provide an effective communication link between patients, providers, and Ryan White program staff. *Aid, coordination and/or completion of all patients' necessary paperwork as it relates to assessments and treatments, individual, group and family therapy, psychiatric care, referrals, and primary care for people living with HIV/AIDS. *This position also gives due consideration to high-risk abuse issues, neglect, domestic violence, exposure to community violence, and/or other forms of trauma often experienced by people living with HIV.

A Therapeutic Assistant is responsible for advocating for patient care as directed by a Multidisciplinary treatment program as well as providing a voice for the patient/victim. *Maintains records of all contacts, referrals, and outcomes with victims; ensure that the Multidisciplinary team is fully aware of the victim's situation; and ensures the con denial handling of all documents or conversations related to victim care. *The Therapeutic Assistant assists with patient admission on discharge processes, monitors and promotes the safety of the therapeutic environment, assure that patient's activities of daily living are maintained, and supports the therapeutic program for individuals in the Military or Civilian sector. * Advocates for the safety and protection of patients and their families in Military and Civilian sector and works to ensure their safety while they are in our facilities as well as transitioning to another facility/home.

Advocated on Students Team to support the goals and objectives of the Initiative while collaborating with local court systems, child welfare, military, and civilian law enforcement agencies on family maltreatment * Worked as a collaborative member of the school district's Student Assistance Team providing professional guidance on intervention strategies for referred students. * Advocated for behavioral intervention to identify by the Student Assistance Team by developing a plan of action to assist students in positive behavioral change. * Provided information and strategies for parents to assist them in confronting the behavioral challenges of their children. * Met with parents, school personnel, law enforcement, Military contacts, and other professionals to address the student safety. * Worked collaboratively with school district Prevention/Intervention Specialist, Site Coordinator, and other district counselors to provide "best practice" programming to students and their families. * Maintained documentation of trainings and report on those conducted.

A high-fidelity Wraparound Care Coordination for families with children and adolescents with complex behavioral health needs both in the Civilian and Military sector the advocates for children/families to ensure growth. *Advocated for Intensive Care Coordination and facilitates team meetings in accordance with high- fidelity Wraparound principles. *Provided intensive Wraparound Care Coordination to children with complex emotional and behavioral needs and their families to ensure they receive all required formal and informal services from continuum of services provided by a variety of public/private agencies, independent professionals, and natural supports available to the families. *Available to provide 24 hours advocacy support families during times of crisis. Scheduled and facilitated monthly Child and Family Team (CFT) meetings as defined by high- fidelity Wraparound standards. *Completed assessments with each youth/family assigned which provided an ongoing basis to monitor youths' progress in care as outlined by agency standards. *Interceded and advocated on behalf of youth and families to ensure appropriate, timely and consistent services, including traveling to, and participating in IEP meetings
