* Monitor, record, and report symptoms or changes in patients' conditions. * Maintain accurate, detailed reports and records. * Record patients' medical information and vital signs. * Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition. * Modify patient treatment plans as indicated by patients' responses and conditions. * Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans. * Provide patient and family education regarding progression in disease process during end of life * Administer medication appropriately for symptom management . * Communicate any changes in patient condition to care team and initiate any changes to care plans if necessary. * Therapeutically listen and communicate with patient and families during emotional times and consult with social work and chaplain services if necessary. * Provide end of life care when necessary. * Build rapport with patients and families. * Perform thorough patient assessment including vital signs, head to toe assessment and focused assessments depending on visit need.
