During the covid pandemic I have been doing house cleaning and private sitting for various people in the Charlottesville area. Duties have included running errands, grocery shopping, banking, meal prep and companion care. Housekeeping has consisted of dusting furniture, vacuum and mopping floors, laundry and basic household duties.

Worked for a high-end family and responsible for overseeing of their residences in addition to three other properties. Duties consisted of house cleaning, laundry, meal prep and occasional child care. Responsible for scheduling and the coordination of vendors for household repairs, yardwork and deliveries. Transportation to and from airport to their private plane.

Assess client's needs and connect them with appropriate services to continue residing in their homes or assist them with placement in an assisted living or nursing facility. Coordinate in home care with local staffing agencies. Work closely with family members, medical staff and other agencies. Strong working knowledge of Medicare, Medicaid, Private health insurance and long-term care policies.

Provide advocacy services to people residing in nursing homes, long-term care facilities, memory care and assisted living facilities. Educate and empower residents and their family members on resident's rights. Interpret federal legislation on nursing home laws. Work closely with the Alzheimer's Association, Elder Rights Law Center, Department of Social Services, Hospital Discharge Planners and Nursing Home Administrators. Managed a Ryan White Federal Grant ($750,000) serving people living with HIV/AIDS. Responsible for annual grant writing, monthly invoicing, account reconciliation and monthly reporting to Virginia Department of Health. Performed professional clinical social work duties in accordance with established policies and procedures. Strongly adhered to confidentiality and HIPAA regulations. Responsible for the delivery of social work in serving as part of an interdisciplinary health care team. Interviewed patients and others to obtain social and medical information necessary to assess the patient's needs and determine their eligibility for various services. Verified financial screenings for numerous programs each with varying requirements. Provided comprehensive casework; discharge planning, and development of treatment plans. Coordinated with other disciplines, social agencies, hospitals, clinics and community resources to meet the needs of patients and their families. Provided education for the prevention of infecting others or re-infection, identify risk factors, and overcoming barriers that prevent one from remaining in medical care. Strong working knowledge of various programs such as Medicaid, Medicare, Affordable Care Act, FAMIS and Social Security Disability.
