Educates on medication, teaches side effects of meds. assisted with medication box set up, Provides dressing changes, wound care, skin care, refills meds if prescribed by provider, calls provider for authoruzation of medication refills uodates pt. med records, documents new med. order communicates with specialty provider relays appropriate outcomes to primary care provider of information received to improve pt. quality care and maintains continuity of patients care .Diabetic blood sugar testing, insulin therapy managment, teaches signs symptoms of hypoglycemia and hyperglycemia episodes how to manage and decrease episodes, teaches importance of taking all prescribed meds as order by medical doctors and why it is so important.
