Care is needed 1 visit per week on Monday, Wednesday or Friday earliest start time is 1pm and the caregiver should be able to start on ASAP. Member would prefer Fridays if possible. This job is approximately 2 hours per week - 6 hrs total. You will play a pivotal role in relaying information back to UHC through the use of digital care plans.
